Home Care can be a combination of skilled and non-skilled services: Skilled home care services include IV therapy, wound care, post-surgical care, rehabilitation and other services provided by professionals. Non-skilled home care services include assistance with personal care or chores. These services do not require a skilled or licensed professional.

You may be eligible for home care services if:
You have had a recent hospitalization or outpatient surgery
You have a short or long term disability
You suffer from a chronic illness

You are experiencing difficulty with aging
You require assistance with managing your home or your personal care needs
Many home care agencies will send a professional nurse or therapist to assess the needs of a patient and create a plan of care with his or her physician.

+ What is the difference between Medicare and Medicaid?
MEDICARE is a federally funded and administered program that provides health insurance for seniors and the disabled. Individuals contribute to Medicare during their working years. Since Medicare is a federal program, the guidelines and services are much the same throughout the United States.
MEDICAID is a health insurance program financed jointly by the federal and state governments. Each state sets its own guidelines, subject to federal rules and regulations. Medicaid is available for low-income people of all ages who do not have money or insurance to pay for health care.
Both Medicare and Medicaid have managed care plans in which members may be enrolled. The individual plan authorizes and manages all services for enrolled Medicare and Medicaid members.

Medicare, Medicaid and private insurance companies will cover some or all of the costs of home health care. We can help you determine if home care services are covered under your insurance plan.
+ Who provides home care services?
There are a variety of home care providers: Certified Home Health Care Agencies
Certified Agencies can bill Medicare and Medicaid and are generally the Agency that handles patients after an acute episode such as a decline in health or a hospitalization.
Individuals requiring skilled services will usually receive this care from a Certified Home Health Agency.
Licensed Home Health Services Agencies
Licensed Agencies provide home care services to clients with private or long term care insurance or who pay privately.
Licensed Agencies also provide aide services to patients of contracted Certified Agencies.
Hospices
Hospices provide home care services to Medicare patients who elect the hospice benefit. These are generally patients at the end of their life who are deemed to have six months or less to live.
Long Term Home Health Care Programs
Long Term Programs provide home care services to Medicaid patients who are medically eligible for placement in a nursing home but choose to receive services at home.
