What these programs are designed to do

NHTD and TBI waiver services are intended to help eligible individuals live in the community rather than in an institutional setting, supporting independence, safety and personal goals.

Service Coordination

Our service coordinator assists the applicant in becoming a waiver participant and coordinates and monitors the provision of all services in the service plan once the individual is determined eligible.

Home & Community Support Services (HCSS)

Our personal care aides provide Home and Community Support Services to a waiver participant who requires assistance with personal care services and whose health and welfare in the community are at risk.

How Summit helps

01 • TALK

Understand the situation

We start with the individual’s current setting, needs, goals and where they are in the waiver process.

02 • IDENTIFY

Clarify relevant supports

Summit helps families understand which Summit services may fit within the participant’s approved plan.

03 • COORDINATE

Put services in place

When Summit is an appropriate provider, the team coordinates the next steps needed to begin support.

Who should contact Summit?
